Declarations

I/We, on behalf of each insured person named in this policy, hereby declare as follows:

 

A. General
1. I/We understand that the policy is only effective after my/our application has been accepted by Allianz Global Corporate & Specialty SE Hong Kong Branch (hereafter "Allianz") and the premium has been paid.
2. I/We hold a valid Hong Kong Identity Card.
3. I/We have read, understood and accepted the terms, conditions and exclusions contained in the Policy Wording and any information or material relating to this Policy.

 

B. Accuracy of Information
1. All information and statements given in the application form for this Policy are true and complete to the best of my/our knowledge and I/we have not withheld any material facts which are relevant to or could affect the acceptance of this application by Allianz. I/We understand that any inaccuracy or non-disclosure may render the Policy voidable. Where applicable, I/we declare that I/we have full and complete authority from the insured person(s) to submit on their behalf this application and disclose any personal information being requested to assess this application.
2. In the event that the statement or information given in the application form for this Policy becomes untrue, incomplete or inaccurate, I/we will immediately inform Allianz before Allianz issues any policy.
3. I/We understand and agree that this application and declaration and other information provided shall form the basis of the contract between me/us and Allianz.

 

C. Other
1. I/We understand and acknowledge that this policy does not cover death or any Injury or Illness directly or indirectly caused by or in connection with any of the following: insanity, suicide (whether sane or insane) or intentional self-inflicted injuries or any attempt thereat; pre-existing conditions; any unlawful act committed or attempted by the insured person; aerial activities; underwater activities exceeding fifty (50) metres in depth; and mountaineering involving the use of ropes or mechanical guides. For full details of all exclusions, please refer to the Policy Wording.
2. I/We confirm that the occupation(s) of the insured person(s) do not fall within the excluded occupations as set out in the Policy Wording (including but not limited to police officers, construction workers and professional sportspersons).
3. I/We understand that if any insured person resides outside of Hong Kong for more than one hundred and eighty (180) consecutive days, only Accidental Death and Permanent Disablement benefits will be payable.
4. I/We confirm that no insured person has any pre-existing conditions that have not been disclosed in this application.

 

Automatic Renewal  

Subject to the Terms and Conditions of the Policy and Allianz’s acceptance of the renewal, the Policy shall automatically renew at the expiry date upon payment of renewal premium, unless a written notice of cancellation has been received by Allianz at least seven (7) days before the expiry date. Allianz reserves the right to revise the renewal premium and/or amend the terms and conditions of the Policy upon renewal at its sole discretion, and renewal is not guaranteed.  

 

Policy Cancellation

This Policy may be cancelled by returning the policy within fourteen (14) days after its delivery or issuance (whichever is earlier) ("Free Look Period") with full refund of the paid premium and levy without interest. After Free Look Period, this Policy may also be cancelled at the request of the Policyholder in writing to Allianz, which the refunded premium and levy will be adjusted in pro-rata based on the part received or retained by the company or the minimum premium. Payment Authorization 

I/We authorize Allianz to collect from my/our designated credit card account all premium and levy in respect of the policy and any renewal thereof. The premium and levy for the renewal of the coming year will be automatically collected from my/our designated credit card account on the renewal date, unless alternative payment instructions have been provided to and accepted by Allianz prior to the renewal date. If the renewal premium payment is unsuccessful for any reason, the Policy will not be renewed and all cover will automatically cease on the expiry date. Allianz is under no obligation to provide notice of an unsuccessful payment.
